- Two Step TB test or X-Ray (within last 6 months)
- Hepatitis B acceptance or refusal
- Criminal Attestation Form AND/OR
- Federal Background Check (PA resident less than 2 years)
- State Background Check (if PA resident for more than 2 years and provides proof of such) *i.e. old pay stubs, utility bills, local tax records, completed tax return, unemployment records, or your ID if the “Date Issued” is 2 years old or older
- Child Abuse History Clearance
- Verification of Driving Status
- Photo ID
- Social Security Card
- Work Visa (if applicable)
